What we receive
What you type into the contact form: your name, email, phone, how you prefer to be reached, a time that suits you, whether you would like to meet in person or by video, and your message. Please leave medical details out of that message.
How we use it
Only to reply to you. We do not sell your information or share it with advertisers.
Your counseling records
We keep your records private and share them only as the law allows. Before you start, Andre explains the limits of confidentiality in writing.
This site
This site has no analytics, no tracking pixels and no chat. The online scheduler and the intake forms belong to the practice's own portal, which has its own privacy notice.
Asking us to delete a message
Write to hello@lumenprovo.com and we delete your form message, except what the law requires us to keep.
